There are NO LONGER any physical pages from the WVREC for this application step because you now file online BUT you still do need two physical forms for upload to the WVREC’s site. One is the affidavit that goes to us step #3 which needs *notarized One of these pages is uploaded in step #9 AFTER you pass the licensing exam (your sponsoring broker form which still needs *notarized)
